You can purchase Patriot America Plus online within 5-10 minutes. You will need to fill out the online application and pay the premium online. Once the application and payment is completed, you will be issued your declaration of insurance which will include your ID card, policy wording, declaration of insurance and other insurance documents. These will be sent to you via email within a few minutes of completing this application online.

Acute Onset of Pre-existing Condition:

ACUTE ONSET OF PRE-EXISTING CONDITIONS: Subject to the applicable Deductible and Coinsurance and the various limits and sub-limits set forth in the BENEFIT SUMMARY, and the Terms of this insurance, including without limitation the CONDITIONS AND RESTRICTIONS subparagraph below, and in the event the Insured Person suffers or experiences an Acute Onset of a Pre-existing Condition during the Period of Coverage for which immediate Treatment is essential and necessary to stabilize the Pre-existing Condition, the Insured Person will be reimbursed up to the amount shown in the BENEFIT SUMMARY for Eligible Medical Expenses incurred during the Period of Coverage with respect to the Acute Onset of the Pre-existing Condition.

To file a claim on your Patriot America Plus Plan, you can log into your member portal and fill out the claim form and attach and supporting documentation from the provider’s office. You can check your member portal for status as well.
